THE FOLLOWING ARE THE PHYSICAL FORMS FOR BOTH HIGH SCHOOL AND MIDDLE SCHOOL ATHLETES:
Sections 1 through 4 - section vii forms. cippe_form_sections_1_2_3_and_4.pdf make sure you have the sport that you are participating in on the physical form.
Section 5 - piaa recertification by parent guardian.pdf *Form should be completed by those who competed in a fall and/or winter sport and have already submitted a physical dated after June 1, 2010
Section 6 - piaa comprehensive pre-participation physcial re-evaluation.pdf * Form should be completed by physician for those who have had an injury between sport seasons.
