Transcript Requests
Alumni Transcript Requests
Please allow two weeks upon receipt of the request for all transcripts to be mailed.
If you are a graduate of Mt. Lebanon High School and would like to request a transcript, please follow these instructions:
- Fill out an alumni transcript release form. Forms can also be picked up in the High School Guidance Office on the 5th floor of the High School
- In your request, be sure to state where the transcript is to be mailed.
- The cost for transcripts is $3.00 for each transcript request. All checks should be made out to Mt. Lebanon High School.
- Mail a SIGNED transcript release form and transcript fee to :
