State Requirements

School physical and dental exams are provided free of charge.

Grade K

1. Certificate of Immunization Card, must be completed & signed by physician or copy of Immunization Record must be submitted.

Emergency Contact Information: Only those persons indicated on the emergency card are permitted to make arrangements for your child's release from school. Updated information is requested yearly and as necessary during the school year. 

Health History: This information is kept confidential and shared with appropriate school and medical personnel as deemed medically necessary.
