Printing Information for
Student Community Service Forms:
There are two student applications forms, the first is to request community service and the second is to report completed community service. The forms can be printed, filled in appropriately and then submitted to Mrs. Kolko in room 648e. These forms are also available in the Community Service office or in the box on the front of the door when the office is not open. If you need more information, students can contact Mrs. Kolko through email at jkolko@mtlsd.net or phone at 412.344.2114 or come to the Community Service Office during office hours, which are posted weekly on the door of the office.
