Transcript Request

If you are a graduate of Mt. Lebanon High School and would like to request a transcript, please follow these instructions:

Questions? Call the Mt. Lebanon High School Counseling Department at 412-344-2003 (option 5).

  • Fill out an Alumni Transcript release form. 

  • In your request, be sure to state where the transcript is to be mailed.  

  • The cost for transcripts is $3.00 for each transcript request. All checks should be made out to Mt. Lebanon High School.  

  • Mail a SIGNED transcript release form and transcript fee to : 

  • Mt. Lebanon High School Counseling Department 155 Cochran Road Pittsburgh, PA 15228  

  • Please allow two weeks upon receipt of the request for all transcripts to be mailed.
