Request A Transcript 

Graduates of Mt. Lebanon High School can request a copy of their transcript. 

1. Download and Complete the Release Form
   • Fill out the Alumni Transcript Release Form. Be sure to include where you would like the transcript mailed.  
      
2. Prepare Your Payment 
   • The fee is $3.00 per transcript. 
      
3. Mail Your Signed Form and Payment 
   • Send the signed release form and check to: Mt. Lebanon High School Counseling Department, 155 Cochran Road, Pittsburgh, PA 15228
      
4. Allow Two Weeks for Processing 
   • Please allow two weeks from the date your request is received for your transcript to be mailed.
