Below you'll find the health requirements, forms, and permissions for your child's time in our schools.
Health Requirements & Information
- New Student Health Packet — A welcome guide to school nursing services and what new families need to complete.
- PA Department of Health Requirements — A grade-by-grade overview of required exams and immunizations.
- Immunization Requirements — Pennsylvania and Allegheny County vaccine requirements for school attendance
Exam Forms — Complete & Return
Required for Grades K, 6, and 11, new students, and students with incomplete records. Return to your child's school nurse.
- Health History — Completed by parent/guardian.
- Physical Examination Form — Completed by your physician (or the school physician with consent).
- Informed Consent for School Physical — Completed by parent/guardian, only if you'd like the school physician to perform the exam at no cost.
- Private Dental Report — Completed by your dentist.
- Private Scoliosis Exam Form — Completed by your physician; for Grade 7. Return by December 1.
Medication At School
If your student needs medication during the school day.
Authorization for Medication — Completed by both parent/guardian and physician. A new form is required for each medication, each change, and each school year.
Permission to Self-Carry & Self-Administer - For students approved to carry and use their own medication. Each requires a completed medication form on file and sign-off from the certified school nurse. All forms should be completed by a parent/guardian.
